Health Partners Medicaid and CHIP Dental
Avesis Third Party Administrators, Inc. has joined with Health Partners of Philadelphia to administer the delivery of dental services to members enrolled in Health Partners' Medical Assistance and CHIP programs starting January 1, 2012. Your participation will allow you to deliver dental services to the members enrolled in the Health Partners and KidzPartners programs.
As the new administrator, Avesis will be responsible for the administration of all aspects of the dental programs noted below, including the dental provider network, eligibility verification and claims processing for covered dental services provided to eligible members.

About the Programs
- Health Partners is one of the Philadelphia area's leading Medical Assistance health plans offering medical care coverage, including dental care coverage to eligible Medical Assistance recipients in the HealthChoices Southeast region (Bucks, Chester, Delaware, Montgomery, and Philadelphia counties).
- KidzPartners, Health Partners' plan for children, is offered through a contract with the Children's Health Insurance Program (CHIP) of Pennsylvania. CHIP is a State- and Federally-funded program to provide health insurance for uninsured children from birth until they reach the age of 19.
If you are not currently an Avesis contracted dental provider and are interested in participating in these programs, please complete the Provider Credentialing and Contracting paperwork by following the link below and mail them to the Avesis address indicated below. Providers must have an NPI number and a State issued identification number (PROMISe number) for each location participating in the program.
