Care Improvement Plus
Avesis currently administers routine vision benefits for Care Improvement Plus Medicare Advantage members located in Arkansas, Georgia, Illinois, Indiana, Iowa, Missouri, New Mexico, New York, South Carolina, Texas, and Wisconsin.
Effective January 1, 2013, Avesis will administer routine vision benefits for Care Improvement Plus Medicare Advantage members located in Alabama, North Carolina, Nebraska, and Pennsylvania.
Members' benefits vary according to their individual coverage. As a participating provider, it is imperative that you verify eligibility and benefits on all members each time they present in your office. Please remember that only the services covered by the member's benefit plan will be paid by Avesis.

Plan benefits include vision services such as routine eye exams, frames, spectacle lenses and lens options. The plan also offers a benefit toward contact lenses and the fitting fee (in lieu of frames and lenses). Benefits may vary depending on the member's plan.
If you are not currently an Avesis contracted provider and are interested in participating in these programs, please complete the Provider Credentialing and Contracting paperwork.
