You can easily check to see what your plan covers, locate a provider, or download a Benefit Summary for your vision plan.

Avesis members have access to one of the most comprehensive vision care networks available. We contract providers from private practice and retail chain locations, Avesis can deliver excellent coverage, high quality of care, and nationwide choices to our members. Once eligible for services, you may choose a provider from the Avesis Provider Directory or any other provider of your choice. When calling to make an appointment, please let them know that you are a member of the Avesis plan. They will ask for your member ID number and then the provider will verify your eligibility.
